Why are women using CBD products — and do they work?

Cannabidiol (CBD) oil and other products containing CBD are being touted as a natural, organic remedy for a wide range of women’s health concerns. Sellers of these products make many claims: CBD has calming effects on sleep, mood, and anxiety; eases hot flashes and improves bone density by balancing hormonal changes of menopause; and has anti-inflammatory properties that clear skin, cure acne, and calm rosacea. It’s promoted for PMS symptoms like bloating and mood swings. And CBD-infused lubricants claim to boost arousal and enjoyment of sex. So, how much of this is true?

First, what is CBD?

CBD is a major ingredient in cannabis plants (like hemp and marijuana). It comes in different strengths and forms, often as CBD oil, but also in pills and powders. It can be absorbed through the skin, ingested, or inhaled. (Vaping it, however, may not be safe, as this blog post and web page from the CDC explain.)

Unlike marijuana, pure CBD products don’t make you feel high. A different ingredient in marijuana called THC makes people feel high.

Does CBD have proven benefits?

So far, there’s not much evidence on the medical benefits of CBD, partly because laws on marijuana made it difficult to study. Until we learn more, it’s wise to keep in mind that few high-quality studies have been done.

  • In 2018 the FDA approved a drug derived from CBD to treat rare forms of childhood epilepsy. This medication was shown in randomized clinical trials to reduce the frequency of seizures (see here and here).
  • A few studies have found CBD may improve anxiety, but the studies were small and of poor quality (see here and here).
  • Some laboratory research on human cells suggests CBD may have anti-inflammatory effects on oil-secreting glands in the skin. This might have implications for acne and other inflammatory skin disorders, but further research is needed to confirm this. And while CBD in skin products is unlikely to harm you, most dermatologists agree that there are more effective and better-studied medications and treatments for acne and inflammatory skin disorders.

Other potential benefits of CBD aren’t clear. No high-quality research shows that CBD improves sex drive, decreases pain, treats depression or mood disorders, decreases PMS symptoms like bloating and cramps, or relieves symptoms of menopause like hot flashes. This may change as more studies are done, but for now, the jury is out.

Are CBD products safe?

The short answer is this: pure CBD seems to be safe for most people. However, we don’t have rigorous studies and long-term data to prove whether or not a wide range of CBD products are safe for everyone. For example, there is no evidence to suggest that CBD is safe during pregnancy or breastfeeding, or for people who are immunocompromised.

Because CBD products aren’t regulated by the FDA in the way that drugs are, there is huge variation in quality and, quite possibly, safety. In 2017–2018, counterfeit CBD oil was found that contained synthetic cannabinoids and led to a poisoning outbreak in Utah.

Testing shows purity and dosage can be unreliable in many products. One study found less than a third of the products tested had the amount of CBD shown on the label. Another study of 84 CBD products bought online showed that more than a quarter of the products contained less CBD than stated. In addition, THC (the component that can make you feel high) was found in 18 products.

Does CBD cause side effects?

CBD can cause side effects like dry mouth, diarrhea, reduced appetite, and drowsiness. Additionally, it can interact with certain medicines, such as blood thinners and antiseizure drugs. If you would like to start using CBD products, it’s best to first talk to your doctor.

The takeaway

There are a lot of extravagant product claims out there about the benefits of CBD for women, but little high-quality research supports them. CBD oil and other CBD products aren’t well regulated. It’s possible what you are buying is counterfeit or contaminated. Before using CBD — especially if you plan to vape or ingest it — first talk with your doctor or healthcare provider to learn whether it could be safe and helpful for you.

I am a 55 year old woman who has suffered with neuropathy since 2004 (amplified by a trauma in 2011); as well as a sciatic nerve issue and other complication since my trauma. One thing I found out (very quickly!), many of the drugs (natural or not) are either recomended for short term relief and used very long term, or the probable cause of added, often more sever, side effects. I don’t believe, for me personally, any medication that has the potential to do more harm than good, especially when it can only treat symptoms and not the cause, would be ideal, unless there is ‘no other option’ or perspective hope. Limited and controlled ecersizes along with diet, seem to have worked best for me personally; but, yes it is very difficult many days. However, I plan to watch my grandchild grow-up, and I plan to do that watching with as clear a mind as possible for today and tomorrow. Side-effects of CBD have been relatively unstudyed or unpublished for lack of verification. That is not promising. All of that being said, I am sure for some people CBD oil could be a God send of relief, most especially for some seizure and cancer patients.

Hormones are essential for every bodily process, including:

What is a hormone imbalance?

Hormones fluctuate and shift naturally throughout the day, during menstrual cycles, pregnancy, puberty, and menopause. These changes are predictable and mean your body is functioning normally.

Other hormone imbalances can happen if one of the endocrine glands produces too little, too much, or even stops producing hormones entirely.

Certain health conditions can trigger hormone imbalances, including:

CBD and the endocannabinoid system

The endocannabinoid system (ECS) is complex. Doctors still don’t fully understand how this cell-signalling system works but know it has three components: enzymes, endocannabinoids, and two types of receptors, CB1 and CB2.

Its primary function is to control how your body releases the neurotransmitters that affect nerve impulses. The ECS could also be involved in regulating:

  • Fertility and pregnancy
  • Sleep
  • Appetite
  • Mood
  • Memory
  • Sensations

CBD, or cannabidiol, is one of the main organic compounds found in the Cannabis sativa plant. The medicinal use of CBD oil is growing in popularity because it confers health benefits without a ‘high’. Taking CBD oil boosts your endocannabinoids so that the ECS can work more effectively.

How does CBD oil influence your hormones?

Research confirms a link between the endocannabinoid system and endocrine processes.

‘We know that the endocannabinoid system has a role in maintaining balance in the body. It makes sure our body is acting within a narrow range of operating conditions; so-called homeostasis.’ explains Dr Moltke.

‘The ECS is known to regulate stress, mood, memory, fertility, bone growth, pain, immune function, among other things. CBD interacts with the ECS and with many other receptors in the body.’

Dr Moltke goes on to explain, ‘We are still highly lacking human studies that show exactly how this manifests in our different hormonal systems.’

CBD can influence major hormones, including insulin, cortisol, and melatonin.

CBD oil and insulin levels

Your pancreas produces the essential hormone, insulin. An imbalance can cause not only diabetes, but heart disease, and obesity.

‘We have long known that marijuana consumption seems to be related to reduced blood sugar levels.’ says Dr Moltke. She goes on to outline that ‘clinical experience and studies are showing that when diabetic people use medicinal cannabis, they can reduce their diabetic medicine or insulin as cannabis lowers blood glucose.

Further research is required to fully understand the effect of CBD on insulin levels. ‘We do not, however, know how CBD alone affects blood sugar or insulin. It probably does not have a direct effect on these measures, but the anti-inflammatory properties can be extremely valuable for type 2 diabetics, as it is a condition associated with chronic inflammation in the whole body due to high blood sugar levels.’

CBD oil and cortisol

Stress is bad for our blood pressure, mental state, and overall health. Cortisol, along with adrenaline, is involved in managing your stress response.

Cortisol is essential for survival. It’s responsible for controlling our natural ‘flight or fight’ response. If you produce too much cortisol, you may suffer symptoms, including weight gain, mood swings, and increased anxiety.

‘One study clearly shows that CBD is able to lower cortisol levels, a hormone associated with chronic stress’ explains Dr Moltke.

CBD oil and melatonin

If you don’t get enough sleep, your health will suffer. Melatonin from your pineal gland is responsible for your circadian rhythm, in other words, when you sleep and wake, and for managing your energy levels. For most people, it’s highest at night and lowest in the morning.

The ECS is involved in your sleep pattern. One study reported that the ECS activated CB1 receptors which affect sleep stability, how quickly you fall asleep and increases the level of melatonin produced.

CBD and hormone imbalances in women

CBD might be able to help with the symptoms of normal monthly hormone fluctuations and those that happen during puberty and menopause.

• Mood swings

Many women experience anxiety and depression as part of their menstrual cycle and during menopause. Oestrogen and progesterone affect levels of serotonin, the so-called happy chemical that influences mood.

Research shows that CBD activates serotonin receptors and that in animal models, it shows anti-anxiety and antidepressant effects.

• Bloating

During menstruation, the uterine contractions that occur throughout your cycle are more intense. This causes food transit through the digestive system to slow down, leading to constipation and bloating.

The drop in oestrogen and progesterone before your period can also cause water retention.

There are cannabinoid receptors in the digestive system, and research confirms that the ECS is involved in the physiological control of colonic motility. CBD could, therefore, help with bloating issues by increasing gut motility.

• Pain and cramps

Some women find the pain associated with menstruation debilitating, others merely uncomfortable. Studies have shown that CBD works to relieve pain and relax cramping muscles.

As progesterone declines, prostaglandins increase, peaking during menstruation. These chemicals cause inflammation, heightened sensitivity to pain, blood vessel constriction, and uterine contractions. Although necessary for a regular menstrual cycle, high levels of prostaglandins can lead to more painful or heavier periods.

An enzyme (COX-2) is responsible for prostaglandin production. CBD inhibits COX-2 and can lower the level of COX-2 that you produce, thereby reducing inflammation, pain, and cramps.

CBD is widely recognised as having muscle and vascular relaxing effects, which also lead to a decrease in painful period cramps.

The way that CBD and the ECS work are complicated and further research is needed, but there is some evidence to suggest that CBD could help with the symptoms of a hormone imbalance.

⚠️ It’s always a good idea to check with your doctor before starting supplementation with CBD. The FSA advises that people who are pregnant, breastfeeding or taking any medication not to consume CBD products. Unless under medical guidance, CBD intake should be limited to less than 70mg a day.

Hormones and CBD

CBD, ECS, and the endocrine connection

CBD indirectly interacts with cannabinoid receptors in the ECS. Because the ECS and endocrine system are closely related, it can also alter the synthesis and secretion of hormones. Now that we have an idea of how CBD can balance hormones, let’s take a look at how it can affect specific ones. Some of the major hormones in our bodies are:

CBD and Estrogen

CBD may lower estrogen levels, but more research is necessary.

What we know: CBD may affect estrogen is through increasing the activity of cytochrome p450, enzymes that commonly break estrogen down. This could be key after ovulation as progesterone surges to assist fertilization.

Estrogen hormones directly affect fertility, sexual development, and possibility of contracting female-related illness like breast cancer .

The hormones LH and FSH promote ovulation and thus the secretion of estrogen from the ovaries. It has been found that CBD may suppress estrogen production through the inhibition of an enzyme called aromatase, which is commonly used to reduce estrogen in women with breast cancer or post-menopausal.

CBD and Thyroid Hormones

Currently there is no research on the impact of CBD and thyroid hormones, but there is information on how endocannabinoids affects them. Thyroid hormones TSH, T3, and T4 all have important functions within the thyroid. Thyroid stimulating hormone (TSH) regulates the production of hormones, while T3 and T4 are connected to weight, energy, hair, skin, nails, and more.

These hormones are active in the use of our metabolism to carry out these functions. Endocannabinoids, however, reduce thyroid hormones, which in turn reduces the energy needed to carry out the necessary functions.

And since CBD can either stimulate or diminish the endocannabinoids, this could have an affect on the thyroid. When endocannabinoids are blocked, this increases TSH, T3, and T4, which could contribute to a more active thyroid.

The effects of CBD may depend on whether the person has an overactive, underactive, or normal thyroid. CBD is known to increase anandamide levels, a fatty acid neurotransmitter. And it has been found that anandamide can diminish TSH in hypothyroidism and normal thyroids, which have high TSH.

It wouldn’t diminish TSH in those with hyperthyroidism however, which have low TSH. This could possibly mean that CBD could adapt based on a person’s needs. More evidence is needed, but the future looks bright for CBD’s treatment of thyroid hormones.

CBD and Testosterone

Along with estrogen, testosterone helps with the growth, maintenance, and repair of women’s bone mass and reproductive tissues. It can also affect behavior. Having an imbalance of testosterone may lead to a decrease in sex drive, particularly in pre- and post-menopausal women.

Testosterone comes from the male and female reproductive organs in response to the hormones LH and FSH from the hypothalamus. High doses of CBD may affect LH and FSH levels, but more evidence is needed to prove it will have an effect on testosterone in turn. Some researchers speculate that CBD may even have a negative effect on testosterone if used at high doses for a long time without exercise.

But CBD may help stimulate testosterone production in other ways. Physical and mental stress can lead to lower testosterone levels. CBD may limit the production of stress hormones prolactin and cortisol which slow the production of testosterone.

CBD and Cortisol

The infamous stress hormone cortisol notifies the body when something stressful is occurring. Cortisol comes from the adrenals in response to sensory inputs from the hypothalamus. Though this hormone may be beneficial in small spikes that occur during an emergency it’s not one we wanted hanging around on the daily.

Anxiety can help us perceive many things as potential danger, such as the possibility of losing our jobs, fights threatening our relationships, or bills piling up. These stimuli can cause chronic cortisol output due to stress that has been unresolved. Known for its anti-anxiety benefits, CBD is the ultimate antidote to stress. CBD has the ability to interrupt the stress response through CB1 receptors in the hypothalamus and Cortisol production is halted as a result.

CBD and Growth Hormone

The growth hormone (GH) is responsible for growth and development in our bodies. This is done through the signaling of cell reproduction and regeneration. Increased growth hormone could lead to stronger bones, more muscle mass, and more energy.

Long term use of CBD may affect GH. It has been shown that a single dose will have no effect on GH levels. However, other cannabinoids such as THC have been shown to reduce GH through its interaction of the CB1 receptor. Since CBD also has an affinity for the CB1 receptor, this could mean it may affect GH as well. Whether or not CBD causes a positive effect still remains to be seen since more research is needed.

CBD and Insulin

Insulin is a hormone which deals with energy and how much is used and stored. It helps with the regulation of our metabolism, which has an effect on weight and energy levels.

A normal insulin level uses sugar efficiently in our muscles and liver, and not much is left to be stored in fat. High insulin levels cause weight gain and diabetes. This is often brought on by high sugar consumption.

Some research shows that CBD can reduce high insulin levels and keep blood sugar low. It may stimulate cells in the muscles and liver to efficiently absorb sugar, thus stabilizing blood sugar.
